“TRT or B12 Included in Membership – No Cost” is subject to provider evaluation and approval. Treatment will only be prescribed if clinically appropriate. Current laboratory testing is required to determine eligibility and is not included in the membership fee. The membership may include testosterone cypionate up to 200 mg per week and anastrozole (either compounded with testosterone cypionate or prescribed separately) up to 10 capsules per month, if clinically appropriate. As an alternative to testosterone therapy, the program may include enclomiphene up to 50 mg daily, clomiphene up to 50 mg daily, or vitamin B12 up to 1,000 mcg weekly, if clinically appropriate. The membership includes either testosterone therapy or one of the alternative therapies listed above; combination therapy is not included. The program does not include testosterone blends, testosterone creams or topical formulations, gonadotropins, or any medications not expressly listed.
